Accident Report Form
Club Activity Factsheet Advice on how to run a club activity which is outside your core activity. It will also guide you through the process of completing a Risk Assessment
Club Activity Registration Form This form will automatically download when clicked. An activity registration form will need to be submitted to the Activities Committee for approval
Club Activity Risk Assessment Template A fully completed risk assessment will need to be submitted alongside you activity registration form. A number of points have been completed for you but please see the factsheet for further help
